HESSCO Elder Services is offering Options Counseling

Services to the residents of Dedham. Individuals who are 60+ or under 60 with a disability are eligible for this service.

· Information on long-term care and choices of settings and services.

· Research and navigation of resources available to help pay for settings and services.

· Discussions about personal goals, functional

capacity, informal and family supports and finances.

· Development of a plan and assistance as needed connection to resources.

· Referrals to organizations, providers and advocacy groups specific to particular disabilities.
